Helping yourself, a family member, a friend or someone in your community to live well improves our quality of life, supporting us to be happier and health
We all have questions about our health, wellbeing, care needs (now or in the future) and how we can connect more. Telford & Wrekin Council have joined forces to pilot and set up the first Community Health & Wellbeing Hub.
A weekly Hub takes place every week in a venue local to you. Regular support includes blood pressure checks, Healthy Lifestyles advice, Independent Living Centre and Telford Mind Social Prescribing.​
